PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY:

 
No matter what type of service you are having The Paw Stop perform for your pet, submitting this application to us is required!  If you have one pet then there are only four pages to the enrollment application. Please fill out all of the appropriate information. The more information that you share with us, the better we will be able to take care of your pet. All fields on the pages that are indicated with a red asterisk are required. 

Page 1 – Pet Parent Information : Here is where you will enter important contact information about you and your partner.  One of the required fields on this page will be an email address.  That email address will help us stay connected with you as well as serve as your Login ID for the website.  You will also need to set and account password which serves two purposes as well.  For one, it will be the password that is required when anyone other than you or your partner pick-up your dog from The Paw Stop.  Secondly, it will serve as your website password.

Page 2 – Pet Information : Please share as much information as possible about your pet.  We ask many questions and some of them might seem a little tedious to you, but we do it so that we can be as informed as possible about your dog.  This helps us in taking the absolute best care of them.  If you have only one dog, then complete the form and click next.  If you have more than one dog, then click add another dog.  It will save the current form and provide you with a new form for your second dog.  You can add up to three dogs.  If you would like to add any more, please contact a Paw Stop representative.  Once you are finished with your pet(s) information, click Next.

Page 3 – Liability Information : This legal form is required.  Read through each of the statements carefully and then acknowledge the statement by entering your initials in the adjacent box.  A digital signature is also required to this page.  Please click the Sign It button.  A pop-up window will appear and you will be required to enter your Mother’s Maiden Name and your Place of Birth.  Enter this information and then click Create Digital Signature.  After you have digitally signed the page, please click next.

Page 4 – Credit Card Information :  Credit Card information is required even if you plan on paying with cash or check.  This credit card will be used to hold reservations, or to make payments if you are a Member, or if your account is past due.  All information is secure and the information on this page is encrypted.  If the address in the credit card is the same, then there is no reason to re-enter the information as it will be pre-populated.  If not, then please enter the correct address.  This form must also be digitally signed.  Since, you will have already created a digital signature on the previous page; all that is required is for you to click the Sign It button.
